People who have a complaint about a business that is violating restrictions put into place because of the coronavirus pandemic can report it to the Pennsylvania Department of Health using an online form.

The form asks for basic contact information from the person making the complaint, information about the business and the nature of the complaint.

Some of the reasons for making a complaint include a business that’s allowing employees to come in sick, not practicing social distancing, not sanitizing and others.

People can also provide other details when making a complaint and they’re asked if they’ve notified any officials about the perceived violations.

The form can be accessed by clicking here.
